Please read these statements carefully: 

• I am a legal parent/guardian or am authorised to register this child(ren) for One80 Camp 2026.
• I understand that all adults and leaders involved with my child(ren) are appropriately trained for safe ministry and for working with children.
• I understand that appropriate safety and risk management measures are undertaken to in an effort to maintain the safety of everyone on One80 Camp.
• I give consent for my child(ren) to be photographed or videoed as needed for church promotional purposes or will contact church in writing if I object to this. 
• I understand that my child(ren)s information provided in this form may be accessed by those in the church community with up-to-date working with children and safe ministry accreditation for the purpose of providing camp services.
• I authorise the leader in charge of the TAC Youth for One80 Camp 2026, where it is impracticable to communicate with me, to arrange for my child to receive such medical treatment as the leader may deem necessary at any time during the time away. 
• I further authorise the use of an Ambulance if in the leader’s judgment it is necessary. 
• I accept responsibility for payment of all expenses associated with such treatment. 
• I give full permission for the above mentioned camper/s to participate fully in all activities associated with One80 Camp 2026. 
• I understand that while  every precaution will be taken to ensure the good welfare and protection of the applicant camper/s, Toongabbie Anglican Church or any leader acting on their behalf are hereby released from any and all liability in the event of any accident or misfortune that may occur to the applicant camper/s or damage or loss to their property. 
• My child agrees to follow the rules and directions given by the leaders and to comply with all safety policies. I agree to my child attending the Camp on this understanding.
